Your use of the any of the facilities indicates your acceptance and understanding of these terms.

	Want to reserve a Tennis Court? It's First Come First Served. The tennis court lights will be turned off when park closes.
	Basketball after dark? The basketball court lights will be turned off when park closes.
	Want to rent the Pavilion? It is available for rental only through the parks and Recreation Office, on Saturdays and Sundays only from 8:30 am - until 1/2 hour before the Park closes. Otherwise the Pavilion is in use during the weeknights for Village meetings. Facility Use Agreements are available in the Park office, or you can save time by downloading one here and filling it out before you get to the office. *PLEASE DO NOT FILL IN THE REQUESTED TIME OR DATE ON THIS FORM. YOU MUST COME TO THE SMITH FARM PARK OFFICE, IN PERSON, WITH THE REQUIRED PAYMENTS TO RESERVE A DATE AND TIME. TELEPHONE RESERVATIONS WILL NOT BE ACCEPTED. To rent the pavilion a check for $200 is needed to secure your date. Dates are reserved on a first pay, first serve basis. Dates will not be held prior to receipt of payment. The payment is comprised of $100 for 4 hours of rental, and $100 as a security deposit. Deposits are refundable within 10 days of your event. Refund of the deposit is conditional upon, no damage to the pavilion, chairs, tables, screening, etc. and it is left in clean and decent shape, the way it was found. All party decorations need to be completely removed from the pavilion to receive a full refund of your deposit. Hanging decorations from fans is prohibited, as decorations can get caught in fan motor causing damage and or potential fire hazard. Use of the pavilion is limited to a maximum of 4 hours per group. Rental of the pavilion includes exclusive use of the covered patio area, bar area, and chairs and tables in covered patio area for the rental period. Rental does not include exclusive use of any area outside of the covered patio area.
	Want to use the Ball Field? The baseball field is available on a first come first serve basis. It's FREE and available for use by individuals or teams.
